Enrollment Process

To be considered for admission into any Medication Certification course, all applicants must complete the required DODD application, meet eligibility criteria, and submit all mandatory documents to the Nurse Educator. Incomplete applications or missing documents may delay or prevent enrollment.

Mandatory Requirements Before Acceptance:

  1. High school diploma, GED, or official college transcript
  2. Completed DODD Application Form
  3. Valid photo identification (Driver’s License, State ID, Passport, or Permanent Resident Card)
  4. Signed Enrollment Agreement
  5. Active and accessible email address

Medina Nursing Services LLC reserves the right to deny enrollment to applicants who do not meet the established admission requirements or fail to complete the required steps.

Payment and Fees

We accept multiple payment methods, including cash, certified bank checks, Cash App, and credit cards. Tuition may be paid on the day of class for your convenience.

Certification Requirements

Medication administration training consists of required classroom hours that must be completed in full. To earn certification, students must demonstrate competency by achieving a score of 80% or higher on a closed-book, multiple-choice examination.

Class Cancellation Policy

If a class must be canceled due to instructor illness, severe weather, or issues with the training facility, you will be notified at the phone number on file. For cancellation updates, you may also contact the RN Instructor at 513-538-3724.

When a class is canceled by the instructor, make-up dates will be communicated via phone or email. Please ensure your contact information is always up to date.

Cancellation and Refund Policy

A full refund of registration fees is available if requested at least three business days before the class start date. Refund requests must be made by 5:00 PM on the third business day via email or fax. Once received, the RN Instructor or office manager will confirm your request.

Approved refunds are issued within 15 days of cancellation and mailed via USPS.
